New patient registration

All UK residents have the right within the NHS to be registered with a doctor. We are pleased to register anyone who lives within the practice area.

When Registering, please bring along with you proof of your ID, for example your driving licence or passport. Please note that we do not keep a copy of this information.

You must provide the name of your previous doctor. If you do not know, or cannot remember, please contact your previous surgery for the necessary information. We need this information to be able to register you with our practice.

You will need to complete an NHS (GMS1) Registration Form for each member of your household which are available to download below, or you can collect from the surgery.

As it takes a while for your medical records to transfer to our practice we also ask you to also fill in our 'personal history' form for each person being registered.

You will need to print a separate form for each member of your household over the age of 16 who wishes to register as a patient with us.

Practice area

The Practice provides general medical services for patients living in the Bransgore and Burley areas. New patients should check with reception to see if they fall within this area.

New patient health check

You will receive a letter by post, asking you to make an appointment with our Practice Nurse for a New Patient Medical. It is important that you attend this as this allows us to collect details regarding past medical health, current problems and medication. Please ensure you bring a specimen of urine when you attend. As for a sample bottle at reception. The information you provide at the health check is essential for the Doctor to continue your health care, as it may take some time before your medical notes reach us from your previous practice.

The appointment gives you a chance to meet us, have your health checked and bring to light any ongoing medical problems or important past medical history, including continuing your medication.

You will have your blood pressure taken and your urine will be tested. Occasionally other examinations or tests may be done if needed.
